HERS LLC Awarded Microgrant

Resilient Futures Co-Lab funds local program to advance healthcare navigation and health equity.

HERS LLC, a client-centered organization focused on healthcare navigation, education, and advocacy to improve health outcomes and reduce disparities, announced that it has been awarded a microgrant from Resilient Futures Co-Lab to support its healthcare navigation and community-based resource coordination initiative.

The funding will support efforts to improve chronic disease management, address social determinants of health, and connect individuals and families to critical healthcare and social resources.

Through personalized navigation, education, and advocacy, HERS LLC works to remove barriers to care and improve outcomes for underserved and marginalized populations. This work contributes to broader efforts to strengthen resilience and equity across the region.

HERS LLC uses a comprehensive, client-centered approach that combines healthcare navigation, case management, education, and resource linking. The organization works closely with individuals, families, healthcare providers, and community partners to improve diagnosis, treatment access, and overall health outcomes, particularly for vulnerable populations, including people of color, uninsured individuals, and those facing complex or symptom-driven conditions.

Resilient Futures Co-Lab is a place-based initiative focused on strengthening the foundations of local economies to support the realization of resilience outcomes across the Mountain and Piedmont counties of North Carolina. It works by aligning capital, relationships, and long-term support around real needs on the ground. Its focus is not on one-off projects, but on how clusters of initiatives, working across different layers of a system, can reinforce one another to unlock durable change.
