Many of the diseases and chronic conditions that affect older adults derive from life-style and environmental factors. Effective health promotion efforts can have a positive impact on addressing these factors and maintaining and/or improving the health and well being of older persons.
The purpose of Health Promotion and Disease Prevention services is to provide programs and activities which will assist older adults in maintaining a healthy lifestyle and/or in identifying health problems or potential problems and to offer effective interventions to address these problems.
A few health promotion efforts in our region are:
Living Healthy with a Chronic Condition OR Diabetes;
A Matter of Balance; and
Guided Autobiography
