The Western North Carolina AIDS Project (WNCAP) is conducting a search for a full-time Case
Manager based in Western North Carolina. A full position description is below.

HIV Medical Case Manager

Direct Supervisor: Case Management Supervisor
The HIV Case Manager is responsible for providing or referring clients to supportive services, as
needed, and submitting appropriate case management documentation.

Specific Duties and Responsibilities:
• Conducts a complete intake and assessment of all new clients.
• Informs clients of their rights and responsibilities and of WNCAP policies and services.
• Determines clients’ eligibility for case management services.
• Provides cutting edge medical and support case management services as appropriate.
• Informs clients of resources and benefits available to them including but not limited to
Medicaid, SSI, Food Bank, Purchase of Care Medicines Program, etc.
• Promotes and respects clients’ right to self-determination.
• Ensures that clients comply with medical care and medication regimens.
• Provides guidance and support to clients in crisis.
• Enters into a contract for case management with the client.
• Follows accepted guidelines for case management, including preparing individualized
care plans, regular re-assessments, and progress notes.
• Assists clients to improve or maintain their quality of life through such efforts as
budgeting, assisting in securing employment or vocational training, providing or referring
clients to counseling or substance abuse treatment.
• Refers clients to appropriate services to address their needs.
• Submits billing records and maintains appropriate documentation for assessing client
eligibility for financial assistance.
• Provides financial assistance for eligible clients.
